School Forms

We are happy to complete school forms for our patients who have seen a provider in last 6-12 months. If your child has not been seen in the last 12 months (6 months with a diagnosis of asthma) we will require the patient to be seen before the forms are completed. Our policy regarding school forms is listed below:

School forms may be picked up at the office or mailed to the patient’s address on file.  If it is not specified, forms will be mailed.

Please drop or mail school forms at any office.

Please make sure we have your current address on file, as this will help speed up the process. 

Please be advised if you submit multiple forms for one child or for multiple children you may receive them at separate times or even on separate days.

School forms will be completed in the order in which they were received and will be returned within 5-7 business days.
